This total failure of care triggered a severe psychotic episode that put him in immediate danger and left our family traumatized. It is irresponsible for a mental health provider to be this careless. I had to manage a life-threatening crisis alone because this office refused to do its job. This isn't the first time they’ve messed up, and they must be held accountable before their incompetence leads to a tragedy.
